Workplace Safety

I work as a safety adviser mainly in the chemical industry and I have been trying to generate a mindmap that covers why we have accidents in industry and the controls that we need to prevent accidents. What a task!! This map goes a small way (every time I look at it, I change it!) in identifying the causes of accidents and the things we need to have in place to avoid them. Nobody wants an accident. Certainly not the injured person. The effects of an accident on a business are huge. Each of the topics in the mindmap can be a mindmap in themselves. I'll be interested to get feedback form other people working in the same environment
